What to expect

Upon separation from employment, regardless of the reason, your benefits will end at midnight on the date of termination.

Medical, Dental and Vision:

In most cases, coverage can be continued under COBRA for up to 18 months.
You will receive information about COBRA from our administrator, Wex Health.

Flexible Spending Accounts (FSA):

You have 90 days from your last day of employment to submit eligible claims for reimbursement. Claims must be submitted for services prior to your last day of employment. Contact Fidelity FSA at 833.299.5089.

Life Insurance and Disability:

You may be able to transition Basic and Optional Life and Accidental Death and Dismemberment (AD&D) coverage (not disability coverage) to an individual policy. Contact Reliance Matrix at 800.351.7500, Group #GL167601.

Voluntary Benefits:

You may be able to convert your coverage for Accident, Critical Illness and Hospital Indemnity to an individual policy within 31 days of your last date of employment Contact Aetna at 800.607.3366, Plan #803122.
