Medicare Transition Services

If you or your spouse are becoming eligible for Medicare DTFOC, has support services to help you understand your options.

When you or your spouse become Medicare eligible, you have the following options:

  • Stay on DTFOC Coverage and Enroll Later
    You may remain on DTFOC coverage and delay enrolling in Medicare. You can enroll later during Medicare’s Special Enrollment Period without penalty when your company benefits end.
  • Enroll in Medicare and Cancel DTFOC Coverage
    You can choose to enroll in Medicare and discontinue your DTFOC coverage. Click here for Workday instructions to cancel your DTFOC coverage within 30 days of the start of your new Medicare coverage. 
  • Keep DTFOC Coverage and Enroll in Medicare
    You may enroll in Medicare while continuing DTFOC coverage. In this case, the DTFOC plan will pay primary to Medicare. Please note: This option requires that you pay premiums for both DTFOC coverage and Medicare coverage. 

Spouse Coverage Considerations

If your spouse is covered under the DTFOC plan, they may choose to enroll only in premium-free Medicare Part A until you retire or you employer coverage ends. 

Enrollment in Medicare Part B—which requires a monthly premium—can be delayed and added later without penalty during Medicare’s Special Enrollment Period, provided your employer coverage is considered creditable. Please note: The DTFOC medical plan is considered credible coverage.

Need a Medicare Form CMS L564 Completed?

Form CMS L564 provides proof of your group health plan coverage based on current employment for Medicare enrollment. 

  • Visit The HUB to submit a request for a completed Medicare Form CMS L564. 
  • Then submit the completed form by mail or fax to your local Social Security office.
