*When you receive services from a non-participating dentist, the percentages in the Out-of-Network columns indicate the portion of Delta Dental’s non-participating dentist fee that will be paid for those services. This amount may be less than what the dentist charges and you are responsible for that difference.

Child(ren)’s eligibility for dental coverage is from birth up to age 26.

The lifetime maximum illustrated is different from the calendar year maximum. For orthodontia services, this limit does not reset each year, this is the most your plan will cover for your services for the lifetime of your participation in this program.
